A "must-have" for every family caregiver! 

Care Consultants  -- experts at the Alzheimer’s Association supporting persons and their families dealing with memory concerns, through education, consultation, assistance with diagnosis and treatments, caregiver support, materials, etc.

Support Planners (Enter "support planners" in the "Enter a Name" field and click the blue "Start a Search" button)  -- professionals trained in person-centered planning and budgeting for buying in-home services, employing support workers, and using home modifications and personal technology.  Skilled in making community living easy, safe, and customized.  Approximate hourly cost:  $45

Long Term Care Consultation Services (Enter "long term care" in the "Enter a Name" field and click the blue "Start a Search" button)  -- provided by counties, helps determine needs, and connects older adults and persons with disabilities to  services and supports. The Senior LinkAge Line® can connect you.

MinnesotaHelp.info®  -- a large on-line database of statewide services and supports for living in the community including rides, meals, chores, senior companion, personal cares, etc., and help for Veterans.
