Who is eligible for VD-HCBS?

 

  • Any age Veteran
  • Needing nursing facility level of care
  • Enrolled in a VA Medical Center
    • Currently in Minnesota, the program is only available to veterans enrolled in the Sioux Falls VA Health Care System and living in one of these Minnesota counties: Cottonwood, Jackson, Lac qui Parle, Lincoln, Lyon, Martin, Murray, Nobles, Pipestone, Rock, and Yellow Medicine
    • If you are enrolled in another VA Medical Center, ask if they have a VD-HCBS Program and express your interest in the program.

    The program may interest veterans and family caregivers who:

    • Find their home care needs exceed the average number of hours generally available through the Homemaker/Home Health Aide (H/HHA) Program at their VA Medical Center
    • Are dissatisfied with traditional agency-based home care providers
    • Want to self-direct and buy individualized services and supports

    Under this option you could buy the following:

    • Help with personal care, taking medications, and meals
    • Rides, errands, and housekeeping
    • Memory care support
    • Personal Emergency Response System
    • Caregiver education, coaching, and relief care
    • Modifying or maintaining your home so your living space works for you
    • Goods to support your independence
    • Adult day services
    • Assistive Technology such as monitoring devices or electronic pill minders
